Paxil (Paroxetine)



The main indication is depression of various origins. Paxil is used for reactive, atypical, post psychotic depression, dysthymia. The drug is effective in case of obsessive-compulsive disorder, panic attacks, social phobias, agoraphobia, anxiety disorders of various origins, nightmares. There is data that the drug is effective for anti-relapse therapy of anxiety and phobic states. There are positive results of treatment with Paxil for patients who have experienced unsatisfactory treatment with standard antidepressants. Paxil should be taken by patients with stress disorders in post-traumatic period. Paxil can be used for a long time, as well as prophylactic.

Drug Contraindications include patients under 18 years, lactation period, patients receiving treatment with MAO inhibitors, pimozide, tryptophan, thioridazine, people who have hypersensitivity to paroxetine, auxiliary substances of the drug.

Method of administration and dosage.

A pill of Paxil is taken in the morning without chewing. It is consumed after a meal (although food does not affect the absorption of Paxil), and washed down with water. A daily dose is taken at one time. The treatment of depression, sociophobia, post-traumatic disorders is operated under the scheme: 20 mg daily in the first few weeks. In case of the ineffectiveness of this dose, you may weekly increase it at 10 mg (for example, in the third week, the daily dose will be 30 mg on the fourth – 40 mg). The maximum dose of Paxil is 50 mg/day.

Overdose Symptoms of Paxil are the following: vomiting, nausea, fatigue or excessive agitation, drowsiness, dizziness, convulsions, urinary retention, cardiac arrhythmias, fainting, confusion, coma, mydriasis, changes in blood pressure, manic reactions, aggression. It can also develop symptoms of liver failure (jaundice, signs of cirrhosis, hepatitis). The co-administered toxic doses of Paxil with psychotropic drugs, ethanol can be fatal. Then you should perform gastric lavage, induce of artificial vomiting, the appointment of adsorbents. In hospital you may have detoxification with intravenous drugs. Supervision is necessary for patient’s vital signs, maintenance of the functions of respiration, cardiac activity. There is no specific antidote.

Side effects include nervousness, emotional lability, insomnia, headaches, migraine, sleepiness, asthenia, tremor, myoclonus, convulsive manifestations of the worsening depression, depersonalization, increased sweating, dry mouth, symptoms of anemia, lymphadenopathy, leukopenia, reduced pressure, orthostatic hypotension (in rare cases), the appearance of hematomas, bleeding, decreased libido, change in fertility due to impaired sperm quality, impotence, disorders of ejaculation, disorders of urination, loss of appetite, disorders of stool, vomiting, nausea, serotonin syndrome, gain of body weight, bile stasis, hepatotoxic effects, sinusitis, rhinitis, fainting swelling of the face, manic disorder, suicidal thoughts, aggression, hostility, galactorrhea, hyponatremia, acute glaucoma (rare), visual disturbances, urticaria, nonspecific rash, photosensitivity, sinus tachycardia. The drug is not prescribed to children because of the presence of significant side effects in this age group: Paxil in the form of self-injurious behavior, increased depression, suicidal thoughts of adolescents.

Use in pregnancy.

The decision to hold a therapy with Paxil during pregnancy is taken by a physician. Paxil is not prescribed to pregnant women in the later stages, as the newborns had such complications as distress syndrome, apnea, convulsive reactions, cyanosis, vomiting, irritability, drowsiness, tremor, instability of temperature, pressure, in the background of reception of Paxil by a mother. Although teratogenic effects were not observed in the experiment, the drug is prescribed to pregnant women in exceptional cases.
