Safety and pharmacodynamics of Modafinil were characterized in several studies conducted outside the USA. In the course of these studies, Modafinil was taken orally in amount up to 4,500 mg in the absence of significant clinical side effects (French, J. Aviation, Space and Environment. Medicine. May 1991, 432). The same study did not report any statistically or clinically significant hemodynamic changes in heart rate or blood pressure of patients or healthy volunteers, which used a test dose of Modafinil.
The manufacturer’s instructions of Cephalon reports that before taking Modafinil it is important to consult a doctor, especially for those who:
has increased sensitivity to the drug or other constituents of the tablets (such as lactose or lactose monohydrate), or suffered from a cardiovascular disease, especially while taking other stimulants, or cirrhosis of the liver, has heart disease, particularly left ventricular hypertrophy or mitral valve prolapse. Modafinil may reduce the effectiveness of certain contraceptives which may result in an unplanned pregnancy. The combination with yohimbine causes a dangerous increase in heart rate and increased blood pressure. The drug should not be combined with alcohol.

- Severe allergic reaction involving the liver or blood cells

- Blistering or peeling of the skin
- Swelling of the face, eyes, lips, tongue, legs or throat
- Problems with swallowing and breathing
